Authorised PGDs for the selected services, with patient eligibility and exclusion criteria and supporting assessment and record-keeping documentation, as applicable.

Communication and reporting

GP communication and notification templates, where applicable, plus adverse-event and clinical-events documentation to support reporting and service monitoring.

Governance and authorisation

Privacy and governance documentation, annual authorisation documentation, and a superintendent pharmacist agreement where applicable. Vaccination PGDs also include an adrenaline PGD for emergency management where clinically indicated.
